Community Rehabilitation Team Health & Social Care Partnership
The Inverclyde Community Rehabilitation Team provides specialist assessment and rehabilitation for older adults with physical disabilities and frailty who require support from a multidisciplinary team.
The team works closely with people and their families and carers , supporting them both in their own homes and in care homes. Operating seven days a week, they play a key role in preventing unnecessary hospital admissions and facilitating early, safe discharge by delivering timely, coordinated care.

The Rehabilitation team consists of:
- Physiotherapists
- Occupational Therapists
- Speech & Language Therapists
- Dietitian
- Podiatrist
- Rehabilitation Support Workers
Following an initial screening assessment, a plan of intervention will be agreed with the service user and carers or family, as appropriate. Rehabilitation is provided within the home.
